Student Forms–These forms are required for your student to start school in the fall. If at any time any of the information on any of these forms changes please resubmit the form(s) to the Front Office. This is a vital step in keeping the children safe and the students files up-to-date and in compliance with state regulations. Unless otherwise indicated, all forms are required for each student.

• Health History
• Routine Screenings Opt-Out Form
• Child Health Report/Assessment (required for students entering Kindergarten for the first time, 1st, 3rd and 6th Grades and all new students. To be completed by a physician and/or transferred from previous school.)
• Dental Form (necessary for children in K, 1st, 3rd, or 6th grades)
• Homeopathy permission
• OTC Medications Permission
• Request for Self Administration of Medication or Administration of Medicine (required by students needing prescription medicines such as inhalers, allergy medication, antibiotics, etc.)
• Rescue Medication Access Permission (required by students needing prescription medicines such as inhalers, allergy medication, antibiotics, etc.)
• Consent and Release Form
• Photo Release Form
• Parental Consent for State Supplies-Text Book Loan Form (required for all new students.)
• School Communication Form
